• Death, 25 Nov 2004, Pretoria, South Africa. David died tragically in the home of his parents at 262 Thatchers Field, Lynnwood, Pretoria at about 10:30 in the morning of the 25th November 2004. From farewell notes he had carefully prepared and typed a day or two beforehand, he was apparently a very unhappy man and at the end of his tether, and had sought his own way out. However, he expressed only deep love for those dear ones he had left behind and bore no illwill to anyone. He was deeply sorry for what he had done and begged their forgiveness.
